Insurance and Payments

I am a preferred in-network provider with the following insurance companies:

• Regence
• Premera
• Lifewise
• First Choice
  • I am not currently seeing patients with Kaiser Permanente insurance, even if it is contracted with First Choice/FCHN.

I encourage you to contact your insurance company prior to your first appointment with me to ensure services will be covered as some insurance companies (including those above) contract with smaller companies that I am not in-network with, and therefore will not be covered by insurance.  Additionally, I am only doing telemedicine visits, with no in-person visits at this time.  It is your responsibility to verify insurance coverage for appointments conducted via telemedicine.  Please be advised that you are responsible for payment of non-covered services.

All copays, coinsurances, deductibles, and private pay amounts are due at the time of service.
